After a fall, 86-year-old Mdm Chan received daily living support and friendship through the enhanced Home Personal Care programme.
After a fall last year, 86-year-old Mdm Chan was enrolled in the enhanced Home Personal Care (HPC+) programme which offered home-based, personalised care services. The programme supports seniors to live well at home and to age in place within their communities.
Under the programme, Mdm Chan was introduced to Ernie from Lions Befrienders, a HPC+ service provider. Ernie often visits Mdm Chan in her home to help with her personal daily living needs, accompanies Mdm Chan to her medical appointments, and even to visit her husband in a nursing home.
The programme is an initiative coordinated by the Agency of Integrated Care and supported by the Tote Board Community Health Fund.
